Cardiac magnetic resonance demonstrates myocardial oedema in remote tissue early 
after reperfused myocardial infarction.

BACKGROUND: Cardiac magnetic resonance can detect myocardial oedema using
myocardial transverse relaxation time (T2)-weighted sequences but quantitative
data are lacking in patients evaluated early after acute myocardial infarction.
AIM: To assess the spatial distribution of T2 in patients with recent acute
myocardial infarction. METHODS: Twenty-four consecutive patients (mean age
60+/-11 years) with acute myocardial infarction (anterior, n=12; inferior, n=12) 
were evaluated prospectively. T2 was determined using a series of breath-hold
T2-weighted segmented half-Fourier turbo-spin echo sequences. No-reflow was
defined as the association of early hypoenhancement and delayed enhancement in an
akinetic region after a bolus injection of DOTA-Gd (0.2mmol/kg). RESULTS:
No-reflow was present in 13 (54%) patients and absent in 11 (46%) patients. Mean 
T2 was increased in the infarct region (84.9+/-23.7ms) compared with in the
remote myocardium (62.8+/-10.3ms, p=0.0001) and in control subjects
(55.7+/-4.6ms, p<0.0001), but also in the remote myocardium compared with control
subjects (p<0.02). In patients with no-reflow, T2 was further increased within
the infarcted subendocardium compared with in patients without no-reflow
(97.9+/-24.8ms vs 76.3+/-24.7ms, p<0.03). Peak troponin correlated with T2
(r=0.47, p<0.02) and was higher in patients with no-reflow (297.9+/-249.7mug/L)
than in patients without no-reflow (42.4+/-43.1mug/L, p=0.003). CONCLUSION: T2
was lengthened in both infarcted and remote myocardium and was influenced by the 
occurrence of no-reflow.

Hereditary thrombophilia and recurrence of ischemic placental disease.

OBJECTIVE: We sought to evaluate whether testing for hereditary thrombophilia
alone or in combination with second-trimester uterine artery Doppler (UAD) is
useful in predicting recurrent complications in patients with previous
preeclampsia, placental abruption, or stillbirth. STUDY DESIGN: Between 2001 and 
2005, 110 consecutive women were included in the study and received 100 mg of
aspirin daily. Adjustment was made for several maternal confounding factors using
a logistic regression model. RESULTS: After multivariable logistic regression
analyses, only abnormal UAD assessment was significantly associated with
recurrent complications (odds ratio, 11.2; 95% confidence interval, 3.8-32.6)
Combining the results of UAD and the presence of laboratory markers of
thrombophilia failed to improve the accuracy of UAD to predict recurrent
complications. CONCLUSION: Hereditary thrombophilia testing is not useful in
predicting recurrent complications in subsequent pregnancy.

Usefulness of multislice computerized tomography angiography in preoperative
diagnosis of ruptured cerebral aneurysms.

OBJECTIVE: Non-invasive imaging methods have become primordial in subarachnoid
hemorrhage. The aim of our study was to evaluate the sensitivity and specificity 
of multislice computed tomographic angiography (MSCTA) for the diagnosis of
cerebral aneurysm. METHODS: The 28 included consecutive patients with SAH
underwent both MSCTA and digital subtraction angiography (DSA). The MSCTA studies
were interpreted by two independent readers (A and B) for the presence, the
location and size of the aneurysm comparatively to the DSA as reference
examination. RESULTS: In 20 patients, 38 aneurysms were diagnosed and in eight no
aneurysm was found. Per patient basis, the diagnostic sensitivity and specificity
were excellent. Per aneurysm basis, the diagnostic sensitivity and specificity of
MSCTA were, respectively, 97.4 and 100% for reader A, 100 and 100% for reader B. 
For aneurysms less than 3mm, sensitivity was 100% for both readers. Interobserver
agreement was excellent for the detection of aneurysm (kappa=0.98, 95% CI
[0.96-1]). Intertechnique and interobserver agreements were excellent for the
measurement of aneurysms (slope=0.86, r=0.91 p=3.1x10(-7) and slope=1.04, r=0.99,
p<10(-6), respectively). CONCLUSION: MSCTA was an accurate and reproducible
non-invasive imaging technique for preoperative diagnosis of ruptured cerebral
aneurysm. The MSCTA may be proposed in first intention after the diagnosis of SAH
was established, with special care regarding injection procedure and a strict
reading method using native images and thin MPR.

Late supra-diaphragmatic lymph node recurrence following resection of a
fibrolamellar hepatocarcinoma: an unusual case.

We report the case of a patient who presented with a supra-diaphragmatic lymph
node recurrence 8 years after resection of a right liver fibrolamellar carcinoma.
Treatment of this recurrence consisted of local excision by a right thoracotomy
approach. Postoperative course was uneventful and the patient did not experience 
recurrence within 2 years. Based on this observation, we describe the major
lymphatic vessels of the liver, in order to explain this unusual metastatic site 
occurrence.

Diagnosis and Monitoring of HIV-1 Group O-Infected Patients in Cameroun.

OBJECTIVE:: To define a routine algorithm for the specific diagnosis and complete
follow-up of HIV-1 group O (HIV-O) infections in Cameroun. METHODS:: During 18
months, samples referred to Centre Pasteur du Cameroun for HIV testing or viral
monitoring were screened for HIV-O infection with an in-house serotyping assay.
HIV-O viral load was quantified by real-time polymerase chain reaction in the LTR
gene and resistance genotyping was performed on pol and env sequences. RESULTS:: 
Of the 7030 samples tested, 78 HIV-O infections (1.1%) were identified, including
7 M and O dually seroreactive samples (9%). All treatment-naive patients and 59% 
of the patients receiving HAART had detectable viral loads. Analysis of pol
sequences from 15 treatment-naive patients revealed a high number of
polymorphisms in the protease region, with natural residues implicated in
genotypic resistance to tipranavir and saquinavir for HIV-1 group M according to 
the Agence Nationale de Recherches sur le Sida et les Hépatites virales
algorithm. Six patients (40%) harbored the 181C mutation conferring natural
resistance to nonnucleoside reverse transcriptase inhibitors. Among
antiretroviral-treated patients, major resistance mutations described for HIV-1
group M were found. CONCLUSIONS:: HIV-O prevalence remains relatively low in
Cameroun. The cocirculation of groups M and O in this country leads to
replicative dual infections. HIV-O-infected patients in this region can now
benefit from effective and specific tools for a complete monitoring of infection.
However, further studies are needed to understand long-term response to
antiretrovirals of these complex variants.

[Palivizumab immunoprophylaxis: Use in clinical practice, safety and beneficial
effects in France.]

BACKGROUND: Infants treated have been followed for one year in order to assess
conditions of use of palivizumab, safety, tolerability, and its impact on
respiratory syncytial virus (RSV) hospitalisation rate. PATIENTS AND METHODS:
Patients who received palivizumab during the epidemic season 2005-2006 were
eligible. Follow-up was carried out 12 months after initiation of prophylaxis.
RESULTS: Sixty-four neonatal level II and III centers, pulmonary and cardiology
units enrolled 1420 children. Mean follow-up was 10.9+/-0.2 months, mean
gestational age (GA) 30+/-4 weeks and mean age at the start of prophylaxis was 5 
months. Median number of injections was 5 and mean time interval between 2
consecutive injections was 30+/-6 days. Treatment was prescribed in accordance
with the marketing authorisation indications (MA) for 84% of patients. For
preterm infants born before 35 SA and less than 6 months of age, 60% was born
before 33 SA and without BDP. The global readmission rate (for more than 24h) for
documented RSV infection during the period of protection by palivizumab was 2.7% 
(37 in 1371) for all treated children: respectively 2% [IC(95%)=1.3-3.2], 2.7%
[IC(95%)=0.7-4.7] and 3.7% [IC(95%)=0.8-6.6] for preterm infants less than 6
months of age, preterm from 6 to 24 months of age and for children with
congenital cardiopathy. Palivizumab safety and tolerability were good.
CONCLUSION: Evaluation of palivizumab prophylaxis in clinical practice confirms
the clinical characteristics of treated infants, outlines their evolution and
confirms safety of treatment. MA were generally well observed and a registry
could be usefull to track the impact of the treatment out of MA.

The Catalogue and index of French-language medical sites (CISMeF) is a medical
portal that provides users with results as pertinent as possible according to
their requirements, expectations, and context of use. Indexing and single-term
research are based on theMedical subject headings(MeSH) thesaurus. The
integration of new medical terminology for indexing the catalogue's resources is 
intended to minimize false-negatives during searches and to contextualize the
users' needs. The creation of a drug information portal makes more targeted
research possible, with numerous entries according to user (physicians,
pharmacists, chemists, and pharmacologists). For simplicity's sake, the
catalogue's index of resources by different nomenclatures is not entirely
displayed. The choice of display is left to the user, with MeSH only as the
default. These multi-nomenclature tools should be applicable as well to
electronic patient records. In this case, the objective is to improve patient
care by better searches and identification of the information required during
consultations and hospitalization.

Dual Effect of Glutamate on GABAergic Interneuron Survival during Cerebral Cortex
Development in Mice Neonates.

In term and preterm neonates, massive glutamate release can lead to excitotoxic
white-matter and cortical lesions. Because of its high permeability toward
calcium, the N-methyl-D-aspartic acid (NMDA) receptor is thought to play an
important role in excitotoxic lesions and NMDA antagonists therefore hold promise
for neuroprotection. We found that, in neonatal mouse cortex, a given NMDA
concentration exerted either excitotoxic or antiapoptotic effects depending on
the cortical layers. In layer VI, NMDA led to excitotoxicity, sustained calcium
mobilization, and necrosis of Gad67GFP neurons. In the immature layers II-IV,
NMDA decreased apoptosis and induced transient calcium mobilization. The NMDA
antagonist MK801 acted as a potent caspase-3 activator in immature layers II-IV
and affected gamma aminobutyric acid (GABA)ergic interneurons. The apoptotic
effect of MK801-induced BAX expression, mitochondrial potential collapse and
caspase-9 activation. In vivo Bax small interfering ribonucleic acid and a
caspase-9 inhibitor abrogated MK801-induced apoptosis and pyknotic nucleus
formation. Ketamine, an anesthetic with NMDA antagonist properties, mimicked the 
apoptotic effects of MK801. These data indicate a dual effect of glutamate on
survival of immature and mature GABAergic neurons and suggest that ketamine may
induce apoptosis of immature GABAergic neurons.

Benefits of ultrasonography in the management of early arthritis: a
cross-sectional study of baseline data from the ESPOIR cohort.

Objective. To assess ultrasonography's (US) performance to detect the structural 
damage in the initial evaluation of early arthritis (EA) using the Etude et
Suivides Polyarthrites Indifférenciées Récentes (ESPOIR) cohort. Methods. ESPOIR 
is a French, multi-centric EA cohort. Four centres assessed the structural damage
by both X-ray and US examination at baseline. X-rays of hands and feet were read 
first by the centre's local investigator (usual reading), then in the X-ray
coordinating centre (central reading). Four trained examiners performed US
blindly from clinical data to detect erosions on the second and fifth MCP (MCP2
and 5) and the fifth MTP (MTP5) joints bilaterally. Results. Patients'
characteristics (n = 126) were: female 78%; mean age 50.3 years; disease duration
103 days; disease activity score on 28 joints 5; CRP level 22.7 mg/l; and 79.4%
of the patients fulfilling RA ACR criteria. Twelve patients had missing data for 
X-rays. US revealed 42 (36.8%) patients with erosive disease, whereas radiography
revealed only 30 (26%) with central reading and only 11% with usual reading. US
missed erosive disease present in X-rays in 10 (8.8%) patients. Combined
technique of both revealed 52 (45.6%) patients with erosive diseases. On the
targeted joints, US detected erosion on 75 (11%) joints vs X-rays on only 11
(1.5%). Only three joints with erosion(s) detected on X-rays were missed on US.
At baseline, the presence of PD activity was not associated with joint erosions. 
Conclusions. US on six joints detected 1.4-fold more patients with erosions
(3.3-fold more with the usual reading). In clinical practice, US combined with
X-rays is of helpful diagnostic value in EA.

Aorto-iliac angiography as a screening tool in selecting patients for
transfemoral aortic valve implantation with the Edwards SAPIEN bioprosthesis.

Aims: Transcatheter aortic valve replacement (TAVR) is a promising therapeutic
option for patients with severe aortic stenosis (AS) considered at too high risk 
for conventional aortic valve replacement. The balloon-expandable Edwards valve
can be implanted using either the transfemoral or transapical routes, depending
on arterial ilio-femoral suitability. The aim of the study was to evaluate the
usefulness of ilio-femoral angiography as an initial screening tool for TAVR
using the transfemoral approach with the Edwards SAPIEN valve.Methods and
results: Aortography with bilateral ilio-femoral run-off was assessed in the
antero-posterior projection. Minimal arterial diameter, vessel tortuosity and
calcification were assessed. Transfemoral implantation was appropriate when the
minimal arterial diameter was >7 mm for a 23 mm trans-catheter heart valve (THV) 
and >8 mm for a 26 mm THV with limited calcifications and tortuosity. Included
were 135 patients with severe aortic stenosis undergoing cardiac catheterisation 
before conventional aortic valve replacement or TAVR. Mean femoral artery
diameter was 7.5+/-1.3 mm and mean iliac artery diameter was 9.3+/-2.3 mm. There 
was no or moderate tortuosity in 72% of them. Calcifications were absent or
moderate in 83%. At first sight, the transfemoral route was considered
potentially suitable in 71% of patients for a 23 mm THV and 39% of patients for a
26 mm TAVI.Conclusions: In the course of haemodynamic and angiographic evaluation
of patients with severe symptomatic aortic stenosis, angiography of the
ilio-femoral access can be used as a first screening modality to determine
potential candidacy for a transfemoral TAVR using a balloon expandable THV.

In vitro secretion of matrix metalloprotease 9 is a prognostic marker in
childhood acute lymphoblastic leukemia.

Matrix metalloproteases (MMPs) are endopeptidases involved in tumor cell
invasion. Childhood acute lymphoblastic leukemia (ALL) is characterized by its
capacity to infiltrate different organs. We analyzed the expression of MMP-2, -9,
-14 and TIMP-1 and -2 in a prospective study on 86 children with newly diagnosed 
ALL (73 B- and 13 T-lineage) and 9 children at relapse with B-ALL. Membrane-bound
and intracytoplasmic MMPs and TIMPs were analyzed by flow cytometry, and secreted
MMPs were quantified by ELISA. In patients at relapse, MMP-14 was present in a
greater proportion of the B-ALL cell population than at diagnosis. In patients
with peripheral infiltration, intracytoplasmic MMP-9 was significantly higher
than in patients without infiltration. ROC curve and Kaplan-Meier curve analysis 
showed that a high secretion of MMP-9 (>2450pg/ml/10(6) cells) was associated
with a lower overall survival rate, suggesting that the secretion of MMP-9 is an 
independent prognostic factor in childhood B-ALL.

Bloating is a common symptom, especially in women. In the clinical practice, it
remains a therapeutic challenge. Since recently, its pathophysiology is better
understood: an impaired transit of gas (particularly in the small bowel) or a
visceral hypersensitivity leading to the induction of an abdominal discomfort
despite a normal volume of gas are two of the main causes, far more frequent than
an excessive production of gas. Moreover, bloating can be related to abnormal
viscera-somatic reflexes promoting both an abdomino-phrenic dyssynergia and the
relaxation of the muscles of the abdominal wall. From a therapeutic point of
view, the efficacy of the gas absorbants remains to be more documented. Besides
the treatment of a constipation and the avoidance of nutrients either highly
fermentable or rich in fructose, other therapeutic options include prokinetics
and drugs acting on visceral sensitivity. Probiotics are another promising
option. In some centers, a non pharmacological therapeutic approach, mainly based
on hypnosis, is discussed.

Recurrent rearrangements in synaptic and neurodevelopmental genes and shared
biologic pathways in schizophrenia, autism, and mental retardation.

CONTEXT: Results of comparative genomic hybridization studies have suggested that
rare copy number variations (CNVs) at numerous loci are involved in the cause of 
mental retardation, autism spectrum disorders, and schizophrenia. OBJECTIVES: To 
provide an estimate of the collective frequency of a set of recurrent or
overlapping CNVs in 3 different groups of cases compared with healthy control
subjects and to assess whether each CNV is present in more than 1 clinical
category. DESIGN: Case-control study. SETTING: Academic research. PARTICIPANTS:
We investigated 28 candidate loci previously identified by comparative genomic
hybridization studies for gene dosage alteration in 247 cases with mental
retardation, in 260 cases with autism spectrum disorders, in 236 cases with
schizophrenia or schizoaffective disorder, and in 236 controls. MAIN OUTCOME
MEASURES: Collective and individual frequencies of the analyzed CNVs in cases
compared with controls. RESULTS: Recurrent or overlapping CNVs were found in
cases at 39.3% of the selected loci. The collective frequency of CNVs at these
loci is significantly increased in cases with autism, in cases with
schizophrenia, and in cases with mental retardation compared with controls (P <
.001, P = .01, and P = .001, respectively, Fisher exact test). Individual
significance (P = .02 without correction for multiple testing) was reached for
the association between autism and a 350-kilobase deletion located at 22q11 and
spanning the PRODH and DGCR6 genes. CONCLUSIONS: Weakly to moderately recurrent
CNVs (transmitted or occurring de novo) seem to be causative or contributory
factors for these diseases. Most of these CNVs (which contain genes involved in
neurotransmission or in synapse formation and maintenance) are present in the 3
pathologic conditions (schizophrenia, autism, and mental retardation), supporting
the existence of shared biologic pathways in these neurodevelopmental disorders.

Synthesis, radiosynthesis and biological evaluation of 1,4-dihydroquinoline
derivatives as new carriers for specific brain delivery.

In spite of numerous reports dealing with the use of 1,4-dihydropyridines as
carriers to deliver biological active compounds to the brain, this chemical
delivery system (CDS) suffers from poor stability of the 1,4-dihydropyridine
derivatives towards oxidation and hydration reactions seriously limiting further 
investigations in vivo. In an attempt to overcome these limitations, we report
herein the first biological evaluation of more stable annellated NADH models in
the quinoline series as relevant neuroactive drug-carrier candidates. The
radiolabeled 1,4-dihydroquinoline [(11)C] was prepared to be subsequently
peripherally injected in rats. The injected animals were sacrificed and brains
were collected. The radioactivity measured in rat brain indicated a rapid
penetration of the carrier [(11)C] into the CNS. HPLC analysis of brain
homogenates showed that oxidation of [(11)C] into the corresponding quinolinium
salt [(11)C] was completed in less than 5 min. An in vivo evaluation in mice is
also reported to illustrate the potential of such 1,4-dihydroquinoline
derivatives to transport a neuroactive drug in the CNS. For this purpose,
gamma-aminobutyric acid (GABA), well known to poorly cross the brain blood
barrier (BBB) was connected to this 1,4-dihydroquinoline-type carrier. After i.p.
injection of 1,4-dihydroquinoline-GABA derivative in mice, a significant
alteration of locomotor activity (LMA) was observed presumably resulting from an 
enhancement of central GABAergic activity. These encouraging results give strong 
evidence for the capacity of carrier-GABA derivative to cross the BBB and exert a
pharmacological effect on the CNS. This study paves the way for further progress 
in designing new redox chemical delivery systems.

[A new approach to antischizophrenic therapeutics: D2 dopamine receptor partial

Since chlorpromazine was described, antischizophrenic therapeutics have been
enriched with various other D2 dopamine receptors antagonists, either
neuroleptics (which develop extrapyramidal side effects) or non neuroleptic
antipsychotics (devoid of extrapyramidal side effects when used at effective
doses against psychotic expressions). The latter drugs display such properties on
account of several associated pharmacological activities (D3 receptors antagonism
more efficient than D2 antagonism; antagonism at cholinergic muscarinic
receptors; antagonism at 5HT2 serotonin receptors). All these antipsychotic drugs
are not, as initially described, neutral antagonists of D2 receptors, but are in 
fact inverse agonists (alpha<0), because they suppress the constitutive activity 
of these receptors (i.e. their spontaneous activity in the absence of dopamine
stimulation). Thus they induce effects which are opposite to those of dopamine.
Partial agonists, also called agonists-antagonists, induce intermediate effects
between those of full agonists and neutral antagonists. Their intrinsic activity 
is the following (1>alpha>0>- 1) in the hierarchy: full agonist>partial
agonist>neutral antagonist>inverse agonist. These partial agonists moderate the
activity of hyperactive dopaminergic neurons (reducing positive expressions:
delusion, hallucinations, agitation). They activate deficient dopaminergic
transmissions, alleviating at least partly negative expressions. This approach is
presently illustrated by aripiprazole: OP 4597. Many other molecules are
candidates for such a therapeutic promotion, justified by the search for both
antiproductive and antideficit activities, with a low incidence of adverse side
effects such as extrapyramidal effects, thermoregulation effects, induction of
late dyskinesias, sedation, bulimia, excessive weight, depressive symptoms,
incitation to consume addictive drugs, hyperprolactinemia....

Apnea training effects on swimming coordination.

Triathletes and elite breath-hold divers show an adaptive response to hypoxia
induced by repeated epochs of breath holding. We hypothesized that hypoxic
training could also improve swimming coordination. Before and after a 3-month
breath-hold training program, 4 male swimmers performed a maximal incremental
test on bicycle and a 50-m front crawl race at maximal speed without breathing so
that interarm coordination could be assessed. Swim velocity, stroke rate (SR),
stroke length (SL), and the arm stroke phases were calculated from video
analysis. Arm coordination was quantified in terms of an index of coordination
(IdC) based on the time gap between the propulsive phases of each arm. After
apnea training, the forced expiratory volume in 1 second was higher (4.85 +/-
0.78 vs. 4.94 +/- 0.81 L, p < 0.05), with concomitant increases in VO2peak,
minimal arterial oxygen saturation, and respiratory compensation point values (W 
and W x kg(-1)) during the incremental test. Swimming performance was not
improved (clean velocity and time on 50 m); however, SR was decreased and SL and 
IdC were increased. These results indicate that apnea training improves
effectiveness at both peak exercise and submaximal exercise and can also improve 
swimming technique by promoting greater propulsive continuity.

Mixed odontogenic tumors include all tumors of the teeth both epithelial and
mesenchymatous. Most of these tumors are rare (ameloblastic fibroma, ameloblastic
fibro-odontoma and fibrodentinoma, odontoameloblastoma, calcified odontogenic
cyst). Other tumors such as odontoma are frequent. They have in common a benign
aspect, their often-fortuitous observation, a higher rate in the young adult and 
surgical treatment by complete exeresis preventing recurrence. They present a
radiolucent lacuna but appear different depending on the presence or not of
intralesional calcifications revealing the production of more or less mature
dental tissue. The diagnosis may be confirmed only by an anatomopathological
examination, along with clinical and radiological observations.

Amniotic fluid embolism is always a serious complication during the peripartum
period. We report the case of an amniotic fluid embolism during curettage for a
pregnancy arrest at 13 weeks. The diagnosis was confirmed by the presence of
epithelial cells into the maternal blood.

Human immunodeficiency viruses (HIV) have a high level of genetic diversity. The 
outlier variants of HIV type 1 (HIV-1) group O are distantly related to HIV-1
group M. Their divergence has an impact on serological diagnosis, with a risk of 
false-negative results. In this study, we report 20 failure cases, involving
patients with primary or chronic infection, in France and Cameroon between 2001
and 2008. Our results indicate that some assays detected group O infection much
less efficiently than others. Two major reasons for these false-negative results 
were identified: the presence or absence of a group O-specific antigen (and the
designed sequence) for the detection of antibodies and the greater envelope
variability of group O than of group M strains. This study highlights the
complexity of screening for these divergent variants and the need to evaluate
test performance with a large panel of strains, due to the extensive diversity of
group O variants.

[Gingivoperiosteoplasty associated to bone graft: radiological evaluation]

INTRODUCTION: Gingivoperiosteoplasty associated to bone graft is part of a
therapeutic strategy applied to the first 20 years of a patient's life.
Management is pluridisciplinary. Most authors recommend a bone graft in mixed
dentition at the end of premaxillary growth. Retroalveolar and panoramic
radiography are the most often used to assess the bone height of the grafted
site. We retrospectively studied the radiographies of 57 alveolar grafts in 44
patients. MATERIAL AND METHODS: Between 1999 and 2005, 44 patients underwent
gingivoperiosteoplasty associated to bone graft. Thirteen underwent bilateral
reconstruction. The surgical interventions were performed by the same surgeon.
One year after surgery, the panoramic radiographies were analyzed by a single
expert. The bone height compared to roots of adjacent teeth was classified in
four grades. Grades 1 and 2 were considered as satisfactory or good and grades 3 
and 4 not satisfactory and an indication for a new bone graft. In case of
bilateral cleft, each side was analyzed independently. RESULTS: Grades 1 and 2
accounted for 84.2% of grafts. There was no statistical difference in alveolar
bone height between patients presenting with agenesis of the lateral incisive.
Eighty-one percent of patients grafted with mixed dentition (66% of the cases)
had satisfactory results (35% of grade 1 and 46% of grade 2). Patients operated
on after 15 years (n=15) had good results in 75% of the cases, 33% were bilateral
cleft patients. There was no statistical difference between patients operated on 
early and those with delayed surgery. DISCUSSION: Radiological results for
gingivoperiosteoplasty associated to bone graft are satisfactory. The procedure
is easy, cheap, and reproducible. Evaluation with panoramic radiography is not as
accurate as with the Denta Scan. CT scan is not used systematically to follow up 
alveolar cleft palate in children so as to limit irradiation. Volumetric
tomography (cone beam) may be the best assessment.

Tumor-infiltrating lymphocytes in colorectal cancers with microsatellite

Colorectal cancers with microsatellite instability are characterized by an
important density of tumor-infiltrating lymphocytes and a good prognosis.
Microsatellite instability results from the inactivation of the DNA mismatch
repair system and induces secondary somatic frameshift mutations within target
genes harboring repeat sequences in their coding frame. By disrupting the open
reading frame, frameshift mutations can result in the appearance of potentially
immunogenic neopeptides. To determine the frameshift mutations inducing a T-cell 
response during the development of a tumor with microsatellite instability, we
studied in 61 colorectal cancer patients with microsatellite instability, using a
fluorescent multiplex PCR comparative analysis, the relative frequency of
frameshift mutations within 19 target genes and analyzed the correlation of these
frameshift mutations with the density of CD3+ tumor-infiltrating lymphocytes. The
four most frequently mutated genes were ACVR2 (92%), TAF1B (84%), ASTE1/HT001
(80%) and TGFBR2 (77%). The vast majority (95%) of the tumors exhibited at least 
three frameshift mutations, and the number of frameshift mutations was associated
with tumor progression (TNM stage, wall invasion and tumor diameter).
Tumor-infiltrating lymphocyte density was associated with the overall number of
frameshift mutations and with the presence of frameshift mutations within two
target genes, namely ASTE1/HT001 and PTEN. These results strongly argue for the
clinical relevance of immunotherapy of colorectal cancers with microsatellite
instability.

Adenosine A2A receptor deficient mice are partially resistant to limbic seizures.

The neuromodulator adenosine, acting through activation of four defined
metabotropic receptors called A(1), A(2A), A(2B) and A(3,) has been proposed as
an endogenous anticonvulsant. Here, the consequences of deleting the adenosine
A(2A) receptor have been examined in different experimental models of epilepsy.
A(2A)R KO mice were not protected against seizures originating from brainstem
structures, namely electroshock-induced seizures. The intensities of seizures
induced by pentylenetetrazol or pilocarpine, as well as the percentages of
convulsing mice, were significantly reduced in A(2A) receptor knockout (A(2A)R
KO) animals. A(2A)R KO mice exhibited reduced pentylenetetrazol-induced kindled
seizures, demonstrating an important role of the A(2A) receptor in the
acquisition of kindling. These data suggest that adenosine stimulating A(2A)
receptors modulates excitatory neurotransmission and exacerbates limbic seizures.
It is therefore suggested that adenosine A(2A) receptor antagonists might offer
protection from some epileptic syndromes.

Precision and recall of search strategies for identifying studies on
return-to-work in Medline.

INTRODUCTION: The purpose of this study was to report on the qualities of various
search strategies and keywords to find return to work (RTW) studies in the
Medline bibliographic database. METHODS: We searched Medline for articles on RTW 
published in 2003, using multiple search strings, and hand searched 16 major
periodicals of rehabilitation or occupational medicine. Among the retrieved
articles, those considered to be relevant, were pooled in a Gold Standard
Database. From this database, we identified candidate text words or MeSH terms
for search strategies using a word frequency analysis of the abstracts and a
MEDLINE categorization algorithm. According to the frequency of identified terms,
searches were run for each term independently and in combination. We computed
Recall, Precision, and number needed to read (NNR = 1/Precision) of each keyword 
or combination of keywords. RESULTS: Among the 8,073 articles examined, 314
(3.9%) were considered relevant and included in the Gold Standard Database. The
search strings ("Rehabilitation, Vocational" [MeSH]), ("Return to work"[All]) and
("Back to work"[All]) had Recall/Precision ratio of 30.46/19.11, 59.55/87.38 and 
3.18/90.91%, respectively. Their combination with the Boolean operator OR yielded
to a Recall/Precision ratio of 73.89/58.44% and a NNR of 1.7. For the end user
requiring comprehensive literature search, the best string was ("Return to work" 
OR "Back to work" OR "Rehabilitation, vocational"[MeSH] OR
"rehabilitation"[Subheading]), with a Recall of 88.22% and a NNR of 18.
CONCLUSIONS: No single MeSH term is available to help the physician to identify
relevant studies on RTW in Medline. Locating these types of studies requires the 
use of various MeSH and non-MeSH terms in combination to obtain a satisfactory
Recall. Nevertheless, enhancing the Recall of search strategies may lead to lower
Precision, and higher NNR, although with a non linear trend. This factor must be 
taken into consideration by the end user in order to improve the
cost-effectiveness ratio of the search in Medline.
